Our asthma nurses will then use this information to review your follow up; if any is needed. The questions have been developed by the Royal College of Physicians and further information can be found on: http://www.patient.co.uk/doctor/management-of-adult-asthma. To complete the questions, please visit our documents page where you can complete the questionnaire, print and return to us at your convenience.
