In order to give you the best travel advice, we have introduced a new system.
If you are requiring advice on travelling abroad, please download the form from our 'Documents & Downloads' page and print off. Alternatively, please contact the Surgery and we will post this out to you.
Complete the form with as much information as you can and return to the surgery at least 6-8 weeks before your travel date. Our nurse will assess your needs and the appropriate amount of time required for your appointment.
Please ensure a contact telephone number is on the form, as we will contact you to arrange an appointment once the form has been assessed.
Please note that not all travel vaccinations are free of charge and sometimes take a few days to be ordered into the surgery. You will be informed of any charges that may be payable and you are not obliged to have those vaccinations with us.
If your trip is last minute, you may prefer to contact a private travel clinic who may be able to see you earlier than we can. Below are the telephone numbers of some centres:
